1675 W. LACEY BLVD 9:00 AM – 4:00 PM

CORCORAN HIGH SCHOOL – SMALL GYM
1100 LETTS AVENUE 9:30 AM – 3:30 PM

AVENAL SENIOR CENTER
108 W. KINGS 10:00 AM – 3:00 PM

ONLY THE FOLLOWING PEOPLE WILL BE VACCINATED
AT THIS CLINIC:

All Children Ages 6 months through 7th birthday
Medically Fragile School-Age Children ages 7-18
People who live with or are Caregivers of Infants under 6 months of age
Pregnant Women

     
  NOTE: 
  • A signed Consent form will be required for every person to be vaccinated
  • The Library, Family Resource Center, or Public Health Clinic in your community will have the Consent Forms or check online at
    http://www.countyofkings.com/Health/H1N1_Influenza.html
  • Be prepared to wait due to the high number of people to be
    vaccinated
  • Bring shot records with you to record the H1N1 Vaccination
  • Second vaccinations will be needed for children age 9 and under
  • Your Healthcare Provider may also have vaccine available
