  • The Library, Family Resource Center, or Public Health Clinic in your community will have the Consent Forms
    or check online at

  • Be prepared to wait due to the high number of people to be vaccinated
  • Bring shot records with you to record the H1N1 Vaccination
  • Second vaccinations will be needed for children age 9 and under
  • Your Healthcare Provider may also have vaccine available
